    There are many key pieces to a successful discharge. PSA offers Care Management services to make sure none of these important pieces get overlooked.

    When you are being discharged from a medical facility the treating physician will write several orders.  Usually orders for in-home services, equipment, and follow up appointments are given to the patient along with a large stack of additional paperwork.

    Returning home, it can be difficult to manage these important services on our own while trying to recuperate.  To ensure a successful discharge it is critical all of the ordered services are effectively managed.  Forgetting to go for scheduled lab work or missing a follow up appointment could lead to a devastating readmission to the hospital.

    PSA will make sure these services and appointments are coordinated.  Often times our clients will request we stay on for the period of time in home services to present your concerns and work with the provider to ensure you stay focused on your recovery.
